Week 5 of Pregnancy: The Neural Tube
Week 5 pregnancy is where the build starts in earnest. The neural tube - the structure that becomes the brain and spinal cord - is forming, and her body has switched into a mode that will flatten her by mid-afternoon. For most couples this is the first genuinely hard week.
- Move anything that needs her attention into the morning and take the evening yourself.
- Claim the whole morning routine permanently, and do not announce that you have.
- Get the insurance and parental leave facts written down this week.
- Leave supplement decisions to her provider and simply keep the supply topped up.
▸The neural tube is this week's headline
The neural tube forms and closes very early in pregnancy, and it goes on to become the brain and the spinal cord. This is why clinicians advise folic acid before conception and through early pregnancy - its role in neural tube development is among the better-established pieces of prenatal medicine. The dose is a conversation for her provider.
The heart is also beginning to form, and the embryo is roughly the size of an air-rifle pellet. All of it runs on a schedule nobody controls, which is worth remembering when you feel useless this week. The biology does not need your input. Her day does.
▸Fatigue that sleep does not fix
The tiredness of week 5 is not ordinary tiredness. Progesterone rises steeply in early pregnancy and has a strongly sedating effect, and her body is simultaneously building an entire organ, the placenta, from scratch. Falling asleep at eight in the evening is a physiological outcome, not a lifestyle choice.
Do not congratulate her on getting an early night, and stop scheduling things for the end of the day. Assume her useful hours have shrunk by several, move anything that genuinely needs her into the morning, and quietly take the evening jobs yourself without turning it into a topic.
▸Run the mornings, without announcing it
The highest-leverage move this week is owning the first ninety minutes of the day: coffee, breakfast, dishes, lunches, whatever your household does. Nausea often lands hardest early, and the gap between waking into a functioning kitchen and waking into last night's mess is larger than it sounds.
Do it without announcing it. A job you claim permanently is worth far more than a favour you perform visibly, because it removes the item from her mental list instead of just moving it across. The point is that she stops tracking it at all.
▸Do the insurance homework this week
This is the unglamorous mission and it pays for itself. Find out what your plan covers for prenatal visits, scans, tests, and delivery; what it excludes; what the deductible or excess is; and how long you get to add a newborn to the policy after the birth. Write it all down.
While you are at it, check what your employer offers on paternity or parental leave and how much notice they require. You do not have to decide anything yet, and you should not announce anything yet either. You do need the facts on paper before appointments start generating bills nobody expected.
▸What she is going through
- Fatigue hits hard. Growing a nervous system is exhausting.
- Nausea may make its first appearance.

▸Questions dads actually ask
Why is my wife so tired at 5 weeks pregnant?
Rising progesterone is sedating, her blood volume and cardiac output are climbing, and her body is building the placenta. That combination produces an exhaustion most people describe as unlike anything ordinary. It commonly eases in the second trimester. Until then, plan around it rather than expecting her to push through.
Can you see anything on an ultrasound at 5 weeks pregnant?
Sometimes a gestational sac and often not much else, and many providers will not scan this early without a specific reason. If she is offered an early scan for dating or because of symptoms, go with her. Seeing less than you hoped at five weeks is common and does not on its own mean something is wrong.
Should she still be taking folic acid at 5 weeks?
That is a question for her midwife or doctor, and it belongs on the list for the first appointment. Folic acid is standard advice in early pregnancy because of the neural tube, and her provider will say what she should take and for how long. Do not pick a product off a shelf on your own judgement.
