Week 4 of Pregnancy: The Positive Test
Week 4 is usually the week you find out. The embryo has implanted in the lining of her uterus and started producing the hormone home tests look for, which means a missed period and two lines on a stick. Whatever you do in the ten minutes after she tells you, she will remember it.
- React out loud when she tells you, and leave the logistics until the next day.
- Book the first prenatal appointment this week - good practices fill up fast.
- Take over prenatal vitamin resupply permanently so she never has to track it.
- Any bleeding, severe pain, or dizziness goes to her midwife or doctor immediately.
▸Implantation, and why the test works now
A few days after fertilization the ball of cells reaches the uterus and implants into its lining. That is the point at which the pregnancy hormone hCG starts being produced, and it is what a home test detects in urine. Levels climb quickly in early pregnancy, which is why a test can read negative one day and positive three days later.
The embryo itself is minute, smaller than the tip of a dart, and buried in tissue. There is no bump, no visible change, and nothing an ultrasound would reliably show yet. Almost all of week 4 is chemistry rather than anatomy, which is exactly why the only evidence you get is a line on a test.
▸The reaction she will remember
However you feel, she is watching your face. If you are thrilled, say so out loud rather than assuming it shows. If you are terrified, that is extremely common and you can say that too, just not first and not instead of the other thing. Lead with her: ask how she is doing before you narrate your own reaction.
Then hold there. Do not immediately start solving, planning, or spreadsheeting - all three are ways of managing your own nerves, and she will read them as distance. Week 4 is not the moment for a project plan. Sit with her for an hour, mark it however the two of you mark things, and let the logistics start tomorrow.
▸What 4 weeks pregnant actually feels like
Often nothing. Some people get light cramping, mild spotting around implantation, sore breasts, or a metallic taste. Plenty feel completely normal. Symptoms at this stage are a poor measure of how a pregnancy is going in either direction, so read nothing into their presence or their absence.
Emotionally, expect the full range and expect it to swing hard. Excitement and dread frequently turn up in the same hour, for both of you, and one does not cancel the other. That is not ambivalence about the baby. It is a normal response to enormous, irreversible news.
▸First admin: appointment, cover, vitamins
Call and book the first prenatal appointment. Depending on where you live and who your provider is, it is commonly scheduled somewhere between weeks six and ten, and busy practices book out. Make the call the week you find out, not the week you feel ready.
Take over the supply chain on prenatal vitamins so she is never the one noticing an empty bottle. Folic acid is what clinicians consistently advise in early pregnancy; the product and the dose are their call, not yours and not the internet's. Then go and confirm what your insurance actually covers.
▸What she is going through
- Missed period, possible light cramping.
- Emotions running the full spectrum, excitement and terror are both normal.

▸Questions dads actually ask
Can a positive pregnancy test at 4 weeks be wrong?
False positives are uncommon. False negatives are far more likely, usually from testing too early or from diluted urine. If a test is positive, treat it as positive and book the appointment. If results are inconsistent, or she has bleeding or pain, call the clinic rather than buying another box of tests.
What should I say when my wife tells me she is pregnant?
Say the true positive thing first and out loud - that you are happy, that you are in this, that you love her. Then ask how she is feeling and listen. Save the questions about money, timing, and logistics for another day. She is not looking for analysis in that moment.
Is cramping normal at 4 weeks pregnant?
Mild cramping is commonly reported in early pregnancy as the uterus changes. Cramping is also a symptom of problems that need attention. Anything severe, one-sided, accompanied by bleeding, or paired with dizziness or shoulder pain gets a same-day call to her midwife or doctor, not a search engine.
