Week 12 of Pregnancy: Reflexes
Week 12 is the one you have both been counting toward. In most places this is when the dating scan happens, and it is the first time you will see the baby move on a screen. Here is what actually happens in that room, what the scan does and does not tell you, and what your job is while you are in there.
- Book the whole half-day off and handle parking, paperwork and payment yourself.
- Check the appointment letter for full bladder instructions before you leave the house.
- Ask before you leave who calls with screening results, and roughly when.
- Ask her how she feels before you tell anyone else the news.
▸What actually happens at the 12 week scan
The 12 week scan, also called the dating scan, is usually an abdominal ultrasound. She lies down, cold gel goes on her lower abdomen, and the sonographer moves a probe over it. Some clinics ask her to arrive with a full bladder because it improves the view, so check the appointment letter rather than guessing.
The sonographer measures the baby from crown to rump to date the pregnancy, confirms how many babies there are, checks the heartbeat, looks at where the placenta has implanted and does a basic structural check. In many systems the nuchal translucency measurement is taken at this same appointment if you have opted into screening.
Two things surprise people. The due date can move: if the measurements date the pregnancy differently from her last period, the official date changes, sometimes by more than a week. And the room is often quiet, because sonographers frequently work without commentary while measuring. If you want to know what they are looking at, you are allowed to ask.
▸What the scan tells you, and what it does not
This scan is primarily about dating, viability, the number of babies and a first look at basic structure. It is not the detailed anatomy scan, which usually happens around weeks 18 to 20 and is where organs, spine, heart and limbs are examined properly. Do not walk out of week 12 believing everything has been checked.
It usually will not tell you the sex reliably either, though some sonographers offer a guess. And screening results may not be same-day: where the nuchal measurement is combined with bloods, the result often comes later by phone or letter. Ask before you leave who calls, roughly when, and on what number, because the waiting is the hard part and knowing the window makes it survivable.
▸Your job in that room
Handle every piece of logistics so she has to think about none of it. Know where you are parking, leave earlier than feels necessary, bring the notes and any ID or payment the clinic asked for, and carry her coat and bag. Block the whole morning rather than slotting it between meetings.
Inside, sit where she can see you and hold her hand rather than your phone. Ask before filming or photographing anything, because plenty of departments do not permit it. Save your questions for the end, once the measuring is finished. Then, when you walk out, ask her how she is before you start texting anyone. She may need ten minutes in the car before she becomes an announcement.
▸The twelve-week exhale, and why it is not universal
The chance of miscarriage falls substantially once the first trimester is behind you and a heartbeat has been seen, which is why so many couples treat week 12 as the moment they are finally allowed to feel it. That release is real and you should let yourselves have it.
It is not zero, though, and pretending otherwise helps nobody. Bleeding, severe pain, or anything that alarms either of you goes to the midwife or doctor at any stage, including after a good scan. That number belongs in both your phones already.
And if this pregnancy follows a loss, week 12 can be harder rather than easier. Anxiety does not switch off on a schedule because a milestone passed, and a good scan does not necessarily undo it. If that is where you are, say so to each other and to the midwife rather than performing an exhale neither of you feels.
▸What she is going through
- Risk of miscarriage drops significantly, many couples exhale here.

▸Questions dads actually ask
What happens at the 12 week scan?
A sonographer applies gel to her abdomen and uses an ultrasound probe to measure the baby from crown to rump, confirm the number of babies, check the heartbeat, look at the placenta position and perform a basic structural check. If you have opted into first-trimester screening, the nuchal translucency measurement is often taken at the same appointment.
Should the dad go to the 12 week scan?
Yes, and block the whole half-day rather than squeezing it in. For most dads this is the moment the pregnancy stops being theoretical, which matters more than people admit. Practically, you are there to drive, carry things, ask the questions she forgets, and be beside her whatever the scan shows.
Can they tell the gender at the 12 week scan?
Not reliably. Some sonographers offer an early guess, but this scan is not designed to determine sex and guesses at this stage are sometimes wrong. The anatomy scan later in the pregnancy, or a blood-based screening test that reports it, is a far more dependable source if knowing matters to you.
Does she need a full bladder for the 12 week scan?
Often yes, because it can improve the view, but it varies by clinic and by how the scan is performed. The appointment letter or the department will say. Check in advance rather than arriving to find she needs to drink half a litre and wait, which is a miserable way to start the morning.
